Impact Statement
This is the first year Blaze of Hope has been in existence.
Accomplishments achieved this year are as follows:
-   Developed organization
-   Obtained 501c3
-   Assisted the Ipe family, our first BLAZE of HOPE recipient
-   Youth volunteers created bracelets for donations
-   Completed first fundraiser 

Top goals are as follows:
-   Develop and grow youth volunteer program
-   Assist additional families 
-   Establish business partners
-   Obtain grants and funding in order to assist families
-   Host at least one fundraising event each quarter 
Needs Statement
Blaze of Hope's top needs are as follows: 
1.  Volunteers - priceless
2.  Marketing/Promotional materials $7,500 (advertising, public relations, print & broadcast, design & printing cost for all print materials such as newsletters, brochures & releases, direct mail costs and thank you cards. Website development & maintenance and special events).
3.  Funds to assist families $67,500  (135 families at $500 per grant) 
4.  Office Supplies $1,500 (ex: computer, printer, paper, etc)
5.  Licensed CPA $1,200.
Background Statement
My name is Lisa and this is my story. 15 years ago I lost my precious baby boy Blaze, to liver cancer. He was diagnosed at just 7 months old and sadly passed away at 13 months. It was BY FAR THE HARDEST time of my life. Because Blaze was so sick, I had to take a leave from my job as we spent most of our time in the hospital. Blaze endured aggressive chemotherapy and underwent multiple surgeries. During his treatments, I was able to stay with him, I was able to be a mom, I was able to sleep in the hospital bed with him nightly, and I was lucky enough to have not been forced to miss a single precious moment of time with him. The provisions for both Blaze and myself were met and I am forever grateful. Unfortunately for some parents, this isn’t always the case. In the days to come, donations were made, fundraisers were had, and even collection jars were on the counters of convenience stores throughout all of Sarasota. All of this was organized and executed by the loving people in our community to help meet our needs. People just like YOU! People who wanted to help... even complete strangers! The compassion, love, support and prayers gave me HOPE in my greatest time of need.

While I was at the hospital I witnessed others who were desperately in need of  HOPE of their own. Not only were they struggling with the unexplainable sadness and pain of caring for their sick child, they were also struggling to meet their financial responsibilities. These people chose their child instead of their mortgage payment. A choice that no parent should ever have to make. Families were torn apart because of difficult choices and the lack of resources available.

My personal experience and short time with my beloved Blaze, has driven this indescribable desire to finding new ways to help others. From that moment on, it has become my dream and lifelong goal to honor my son by helping as many people as I can whose children are ill. Glory be to God for giving me the insight and strength to develop "Blaze of Hope", a non-profit organization in which a family can simply complete an application and funds from Blaze of Hope will be appropriated to assist during the wearisome times of having a sick child.

Description Youth Board will be an opportunity for middle and high school age students to directly influence certain operations of Blaze of Hope. Monthly meetings are held to discuss and practice ways to engage others in their age group.

Short Term Success Youth Board was formed after 1 young lady had an idea. Since then we have added 4 members. All youths are required to complete a 1 hour orientation session. The hope is that they learn the basics of fundraising, life skills (proper phone calls, mailings, etc. ), proper etiquette while in public and the importance of what we are doing within Blaze of Hope.
Long Term Success 100% of our teens will graduate from Youth Board upon reaching their 20th birthday and will transition into Adult volunteers with the potential to become Blaze Of Hope Board members.
Program Success Monitoring Youth Board is overseen by Adult Board members Lisa Moore and Theresa Millett. Meetings are held on a monthly basis to discuss strategies, concerns and functions of Blaze of Hope.
Program Success Examples 1 young lady had an idea to make and sell bracelets as a fundraiser. 2 other girls pitched in and 4 hours later, the 3 of them raised $300 selling bracelets for $5 each.
