Impact Statement
1. Completed the first year of our three-year FY15-17 Strategic Plan ( including Program Outreach, Advocacy, Research and Development) meeting or exceeding the FY15 goals.
2. Secured State funding to purchase a new mobile office, the Memory Mobile, to assist in providing outreach and services to isolated and underserved people living with Alzheimer's disease and their caregivers.
3. Implemented programs specifically targeting female caregivers; who were identified in the 2014 Alzheimer's Disease Facts & Figures Report as most significantly impacted by the disease.
2016 GOALS (based on Strategic Plan):
1. Increase Alzheimer's concern and awareness through increasing media impressions and participation in Association activities.
2. Advance public policy initiatives on regional, State and Federal levels by increasing federal funding for research, creating early detection initiatives, care planning initiatives and developing a Federal and State Alzheimer's plan.
3. Enhance earlier access to care and support through physician outreach and access to services by newly diagnosed individuals and their caregivers.
4. Accelerate research progress through increasing research funding and earlier access to clinical trials.
5. Achieve significantly greater Chapter mission impact by growing combined events revenue by 45%, individual gifts by 17%, corporate and foundation gifts by 19%  and grant revenue by 30%.
Needs Statement
  • Ability provide our Caregiver Connection Program to an additional 750 people living with Alzheimer's disease and their caregivers.   The Caregiver Connection is a coordinated array of services (including but not limited to Care Planning Consults, Caregiver Training, Support Groups, 24/7 Helpline, and Respite Care) designed to address the unique support needs of the target populations.  Cost $45,000
  • Provide 30 Medic Alert5 +Safe Return scholarships to people living with Alzheimer's disease to help locate them and return them safely home should they wander and become lost. 80% of people living with Alzheimer's disease will wander and become lost, if not found within 24 hours 50% will be seriously injured or die. The program has a 99% success rate of returning wanderers safely home. Cost: $1,890
  • Duplicate and package 400 DVDs that will be provided to physicians to distribute to newly diagnosed individuals and their caregivers to inform them about the disease and the support services that are available to them. This information will streamline access to support programs that can prevent costly mistakes as they learn to cope with this new hurdle in their lives.  Cost: $2,000
  • 24/7 Helpline services for 300 individuals providing them information on all aspects of Alzheimer's disease; including the complimentary mailing Helpline packets of information tailored to their specific needs. Cost: $1,440
Background Statement

The Florida Gulf Coast Chapter was founded in 1982. Known then as the Tampa Bay Chapter, it served Pinellas, Hillsborough, and Polk counties. In 1999, the West Central Florida Chapter was merged with the Tampa Bay Chapter adding Citrus, Hernando, Pasco and Sumter counties increasing the Chapter's service region to seven counties.  In 2002, the Sarasota/Manatee Chapter and the Southwest Florida Chapters were merged into the Tampa Bay Chapter. These mergers added Charlotte, Citrus, Collier, DeSoto, Glades, Hardee, Hendry, Highlands, Lee, Manatee, and Sarasota counties to the Chapter's service region.  With this expansion, the Tampa Bay Chapter changed its name to the Florida Gulf Coast Chapter to better reflect its comprehensive 17-county service region. Within the 17 counties that comprise the Florida Gulf Coast Chapter, the Florida Department of Elder Affairs estimates that in 2014 there were 175,902 people living with Alzheimer's disease. This number does not include the “snowbirds” who reside in the region during the winter months. 

Our professional staff is dedicated to helping people navigate through the difficult decisions and uncertainties people with Alzheimer’s and their families face at every stage of the disease. Core services include a 24/7 helpline, care consultations, caregiver support groups, Medic Alert and Safe Return, and education/outreach regarding Alzheimer’s disease. The Chapter also features an advocacy program in which volunteer advocates take issues of concern directly to their legislators in Tallahassee and Washington D.C. to find better medical treatments and a cure. All of the Chapter’s services are provided free of charge and are funded through the Chapter’s grants program, and annual giving campaign, and other fundraising efforts.

 I am proud to be a small part of a tremendous team of professionals and volunteers that have made it their life work to support families living with Alzheimer's disease and other memory disorders.

We have the rewarding opportunity to witness, on a daily basis, a family’s transition from fear and confusion to very visible signs of relief that they are not alone in the devastating journey caused by Alzheimer’s disease. 

 The number of families that need our assistance would be daunting if not for the knowledge and dedication we have to support our caregivers and their loved ones.  We know that through our array of services that families provide better care for their loved ones and that their own mental and physical health benefits. Services that include a 24/7 Helpline, individual and family Care Consultations, Support Groups, Caregiver trainings, Respite care, Early Onset Groups and much more. Each family receiving support to their unique needs.

 One in 8 persons over 65 and nearly half of all those 85 and older are developing Alzheimer’s disease. Our organization through the support of our communities is making sure we are prepared to address the increasing demand for help.   It is a rare occurrence to meet someone that has not been touched by this disease.  However, we stay very hopeful.  The Alzheimer’s Association in addition to supporting families is also the largest private non profit funder of Alzheimer’s research.  Our Association holds an annual international research conference bringing thousands of scientists and researchers together with the sole goal of destroying a disease that is taking away our loved ones and their memories with them.  We want all to know that until there is a cure we will be by your side providing support and care.

Long Term Success

 The Caregiver Connection is a coordinated array of services designed to address the unique support needs of people living with Alzheimer’s disease or a memory related disorder and their caregivers. The program assists caregivers in overcoming barriers and guides them through the process of identifying and accessing support services that assist them in acquiring a higher level of care and life quality for themselves and the loved one for whom they provide care.


Program Success Monitoring The evaluation process is centered on client feedback through satisfaction surveys. Each program’s Satisfaction Survey asks specific questions which will indicate if the participants of that program are receiving the program’s desired outcome. For example, Satisfaction Surveys for Caregiver Trainings will solicit feedback that will indicate if the participant feels better equipped to meeting the needs of the individual for whom they provide care after participating in the training.Based on this feedback we are able to determine if our programs are producing the desired outcome. If the satisfaction surveys would indicate that we not achieving the desired outcome for a specific program, the information collected by the Satisfaction Surveys will alert us to this and provide us with information as to where the program has missed the mark and how it needs to be modified. This system provides us with an on-going method to evaluate program effectiveness and prevents us from unknowingly providing a program that is not achieving its intended outcomes.

1. 96% of Helpline callers indicate they would be comfortable accessing additional services.

2. 87% of respite recipients indicate a feeling of decreased care giving burden.

3. 91% of Support Group attendees report an increase in support through interaction with peers.

Short Term Success Conduct outreach, consisting of at least two contacts, with the goal of reaching the clinical practices of 80 percent of neurologists and geriatricians and 50 percent of the top prescribingclinical practices within general/family medicine, internal medicine and psychiatry over the next three years.
Long Term Success The Alzheimer's Association will be an early and ongoing point of contact for at least 50 percent of people newly diagnosed with Alzheimer's disease, either directly or through a caregiver

 Improve meaningful engagement of early-stage constituents through a 50 percent increase in participation in consistent, high-quality and evidence-based early-stage programs, support groups and volunteer activities.


Program Success Examples
This program has been in effect since July 2011. Measureable in the first six months of the program include identifying and mailing infomration to the top dementia medication physicians in the area, conducting in person outreach to the physician and the development of of data tracking mechanisms to monitor the programs progress towards its mezsurable outcomes.
This program is specifically designed to meet the special needs of indivudals suffering from Younger Onset (20 - 60 years old) and their caregivers in addition to those who are in the early stages of the disease.

Experience Gloria J.T. Smith has 34 years of non-profit management expertise. She has been the President/CEO of Alzheimer’s Association – Florida Gulf Coast Chapter since 2000. She has been an executive with the United Way system, where she was Director of the Management Assistance Program for nonprofits and responsible for planning, evaluation and allocations for member agencies. She was also a nationally known administrator of an AIDS organization that was selected by the U.S. Assistant Secretary of Health as one of the top five AIDS organizations in the country. She has served on numerous of local, state and national boards of nonprofits including serving as Chairperson for the International AIDS Committee of the National AIDS Council.
